Sparkdo.Authorization

提供宿主无关的授权决策运行时，并通过 package-default 自动把 AuthorizationMiddleware 接入 Sparkdo Invocation。

只有明确 Permit 才会执行目标。未知策略、缺少 Handler、配置冲突和运行时故障均 fail-closed。

只有传入授权调用的 CancellationToken 已请求取消时，OperationCanceledException 才按调用方取消原样传播。Policy source、CanHandle 或 Handler 因其他 token 自行取消时，分别闭锁为 PolicyResolutionFailed 或 HandlerFailed。
