- [Sg] Heap: MoltenVK GPU-hang fix — derive-output/alignment holes broke upload-region merging, so a bulk flush issued O(parts) VkBufferCopy regions in one command (each = a Metal blit command -> watchdog). Holes are now staged as zero placeholders: 700k regions -> 12; also caps regions per copy at 16k and logs region counts. Bonus: ingest 11.2 -> 8.6 s @700k, mac upload 8x faster.
- [Sg] Heap: fixed a double dispose of the derive-records buffer in bucket teardown (the long-standing "negative reference count (-1)" warning).
- [Vulkan] AARDVARK_TRACE_RESOURCE_REFS=1 records per-resource creation/addref/dispose stacks and dumps the history when a reference count goes negative.
