Scope Support

PolicyEnforcementGuard now evaluates policies using the scope data available on ToolExecutionRequest:

TenantId
AgentId
ChannelId
UserId

If only UserId is present, it still enforces user-specific and global policies correctly.

  ├─ ApprovalRequired → REQUIRE_APPROVAL
  └─ Enabled/Inherit → ALLOW
